Psychological Evaluation Fee
Fees typically range from $900 to $1500 depending on the complexity of the referral question. An early entry eligibility evaluation is typically $500. Gifted evaluations are $400 to $800.
Neuropsychological Evaluation Fee
A comprehensive neuropsychological evaluation is typically $3,000-$4,000.
Do I need a Referral from my physician for testing?
Billing
We bill insurance companies as a courtesy. We are preferred providers for TriWest. Dr Lawrence Norton is a Medicaid/Denali Kid Care provider as well.
Will my insurance cover these services?
It depends. You
should contact your insurance company to determine coverage, and to determine if pre-authorization is required. Click here before you call your insurance company to determine what they may or may not cover for testing or counseling.
If calling your insurance company regardings skype services, be sure to ask about `telebehavioral health/skype ' codes: `90806- GT, and 90801 GT, 908047 GT'.
Do I need Pre-authorization for testing?
Depends on your insurance.-but you can discuss this with Dr Norton at the Pre-Evaluation/diagnostic appointment. Dr. Norton can then provide you with specific pre-authorization information for your insurance company.
Payment Policy
For Evalutions: Clients
pay 50% of the evaluation fee at the first testing appointment, with final
payment due at the post testing session, unless insurance payments are pending.
For Counseling: Payment is required at conclusion of each counseling session via credit card, check or cash, except for Triwest, which is billed directly. Insurance is billed for you, and will often reimburse you for the majority of counseling appointments.
